We stand out from the rest with two board-certified female doctors, Dr. Fahima Azizi and Dr. Rita Kapoor, who have dedicated their life's work to healing patients and restoring lives. Our doctors specialize in Internal Medicine with extensive training in treating a variety of acute and chronic health conditions.

Our Doctors
Dr. Kapoor and Dr. Azizi are both board-certified in Internal Medicine. Our team has years of experience managing acute complex conditions to chronic medical conditions such as hypertension, diabetes, coronary artery disease, congestive heart failure, and mental health, to name a few.
Fahima Azizi, MD - Internal Medicine
Dr. Azizi attended the Medical University of the Americas in 2010 and completed her residency at UPMC Pinnacle Health System in 2013.
She is fluent in English and Persian and concentrates on preventative care, hospital medicine, chronic medical conditions, obesity medicine, and women's health. When she's not at the office, she's either cooking or enjoying time with her family.
Rita Kapoor, MD - Internal Medicine
Dr. Kapoor is a board-certified medical doctor with many years of experience. Dr. Kapoor pursued her doctoral degree at Dayanand Medical College and Hospital in 2008 and finished her residency in Internal Medicine at Jamaica Hospital Medical Center in New York in 2014.
Her clinical interests include preventative care, chronic medical conditions, geriatrics, obesity medicine, and women's health. She can speak English and Hindi and enjoys reading, tennis, and hiking in her free time.

Our doctors are experts in their field and can provide a wide range of medical services to patients of all ages. We are your one-stop shop for all your direct primary care needs, possessing all the necessary in-house diagnostic tools for successful diagnoses.
Direct Primary Care
Direct Primary Care (DPC) puts us and our patients in charge of healthcare decisions by removing insurance from the doctor-patient relationship. It allows us to make high-quality primary care accessible for the uninsured and under-insured members of our community.
Annual physical examinations
Our team provides thorough physical exams (including USCIS) and other annual routine checkups to maintain your health. Same Day and Next Day Appointments Available!
*There is a $25 holding fee when scheduling immigration appts"
Vaccinations
We administer vaccinations to adults to reduce and eliminate the risk of contracting dangerous diseases. These vaccines include but are not limited to the flu, Tdap (for whooping cough), MMR (for measles, mumps, and rubella), and Td (for tetanus and diphtheria).
In-house Testing
We can handle laboratory testing for urine toxicology, blood chemistry, pharmacogenetic testing, cancer genetics, cardiomyopathy genetics, diabetes and obesity genetics, Alzheimer's and dementia genetics, respiratory pathogen testing, COVID-19 testing, wound panel, and UTI panel tests.
Women's Health
With Elite Primary Care Group, women receive the comfort of receiving care from female doctors who have a first-hand understanding of women's health. Dr. Azizi and Dr. Kapoor treat various female patients, but does not take patients under 18 years of age.
Chronic Care Management
CCM services are non-face-to-face services provided to Medicare beneficiaries with at least two or more chronic conditions expected to last at least twelve months.
These services are outside of the regular office visits for the patients.
Remote Patient Monitoring
Remote patient monitoring (RPM) uses the latest information technology to gather the patient's health data from the comfort of their homes.
Weight Loss Management
In addition to healthy eating, physical activity, and talking to your health care provider, a plan tailored to your specific needs is important. It can help prevent the weight you lose from coming back. The first step to success is exploring your treatment options.
In-house Medication Dispensing
Ever-rising drug costs are a huge problem for many Americans and a major reason for patient non-adherence to treatment recommendations. For your convenience, we provide In House Medicine Dispensing, which often can save you money and improve medication adherence.
